ACORD 125: Filling the Prior Carrier and Loss History Sections
The ACORD 125 is the Commercial Insurance Application — Applicant Information Section, the base application that line-of-business sections attach to. The account's loss runs inform parts of two of its sections: Prior Carrier Information and Loss History. This guide walks through both, field by field, including what loss runs will not supply.
ACORD 125 at a glance
| Form number | ACORD 125 |
| Title | Commercial Insurance Application — Applicant Information Section |
| Current edition | 2025/03 (earlier: 2024/11, 2016/03) |
| Role | The base application; line sections such as the ACORD 126 (general liability) attach to it |
| Sections loss runs inform | Parts of Prior Carrier Information and Loss History |
The 2024/11 and 2025/03 updates changed page 1 and the state fraud warnings. The prior carrier and loss history sections are the same as in 2016/03, so instructions written for the older edition still apply to them.
Prior Carrier Information, field by field
A grid by policy year, with a column each for General Liability, Automobile, Property and Other, continued on the following page for more years.
| Row | Where it comes from |
|---|---|
| Carrier | The issuing company on each year's policy, as shown on the loss run or declarations |
| Policy Number | Each year's policy number. Loss runs list it per policy period |
| Premium | Each year's declarations page |
| Effective Date / Expiration Date | The policy period on the loss run |
Fill one column per line for every year the carrier asks about, and include each prior carrier. If the account moved carriers, that means a loss run from each one. See how to get loss runs. If you are taking the account over from another agent, the broker of record change usually has to clear before those carriers will release loss runs to you.
Loss History, field by field
The section asks for all claims or losses, regardless of fault and whether or not insured, plus occurrences that may give rise to claims, for the last ___ years. It has a "check if none" box and a note to attach a loss summary if you need more room.
| Column | What goes in it |
|---|---|
| Date of Occurrence | The date of loss |
| Line | The line of business the claim was under |
| Type / Description of Occurrence or Claim | A short description: the loss type, cause or injury |
| Date of Claim | When the claim was made or reported |
| Amount Paid | Paid to date |
| Amount Reserved | Outstanding reserve on open claims |
| Subrogation Y/N | Whether subrogation applies |
| Claim Open Y/N | Open or closed status |
| Total Losses | The total across the claims listed |
Paid and reserved amounts change as claims develop, so take them from currently valued loss runs, not a copy pulled for last year's renewal.
Filling it from a loss run CSV
LossRunGuru is built to turn a carrier loss run PDF into a CSV of claims, emailed to you after you confirm your address. It does not fill the ACORD 125. You copy across, and you check every value against the loss run before it goes into the application. The CSV columns that line up with the 125:
| ACORD 125 field | Closest CSV column |
|---|---|
| Carrier | Carrier |
| Policy Number | Policy Number |
| Effective / Expiration Date | Policy Effective Date / Policy Expiration Date |
| Line | Line of Business |
| Date of Occurrence | Date of Loss |
| Date of Claim | Date Reported |
| Type / Description | Loss Description, Injury Type |
| Amount Paid | Total Paid |
| Amount Reserved | Total Reserve |
| Subrogation Y/N | Subrogation Recovery (an amount, not a yes/no) |
| Claim Open Y/N | Claim Status |
Two gaps to know about. Premium is not in the CSV export, so the Premium row of the prior carrier grid comes from each year's declarations page. And a column is only as complete as the loss run behind it: if a carrier's report leaves a field out, the CSV cell is empty. The CSV is available to everyone; Excel output is on paid accounts. See pricing and how it works.
What loss runs will not tell you
- Losses that were never claimed. The section asks for losses "whether or not insured". A carrier's loss run lists only claims reported to that carrier, so uninsured or self-handled losses have to come from the insured.
- Occurrences that have not become claims yet. Same reason: ask the insured.
- Years with no coverage. There is no loss run for a period with no policy. Check the carrier's submission requirements for what it wants for that period.
- Declined, cancelled or non-renewed coverage. The 125 asks about the prior three years separately, with a reason. The loss run will not answer that for you.
Tick "check if none" only when the loss runs and the insured both confirm there is nothing to report.

Frequently asked questions
What is the ACORD 125?
The ACORD 125 is the "Commercial Insurance Application — Applicant Information Section". It carries the applicant, business and policy information for a commercial submission, including the prior carrier and loss history sections, and the line-of-business sections (such as the ACORD 126 for general liability) are attached to it.
What is the current edition of the ACORD 125?
ACORD's forms index lists 2025/03 as the current edition. It replaced 2024/11, which replaced the long-running 2016/03 edition. The prior carrier and loss history sections did not change in either update; the changes were to page 1 and the fraud warnings.
How many years of loss history go on the ACORD 125?
The form leaves the number of years blank ("for the last ___ years"). The carrier's submission requirements set it, and five years is a common request.
Can I attach loss runs instead of filling in the loss history?
The form itself says to attach a loss summary for additional loss information. Whether the carrier accepts attached loss runs in place of the table is the carrier's decision, so check its submission requirements.
Does LossRunGuru fill out the ACORD 125?
No. LossRunGuru is built to turn carrier loss run PDFs into a CSV of claims, emailed to you. You transcribe the relevant columns into the ACORD 125 and check them against the loss runs.
Where can I download a fillable ACORD 125?
ACORD forms are copyrighted, so we do not host one. Get it from ACORD.org with a member account, from your agency management system's forms library, or from the carrier's agent portal.
