Notes

GUARD is one of the few carriers that publishes its loss run routing outright, on its Loss Control page: policyholders download loss runs themselves from the Policyholder Service Center, appointed agents download them for their clients from the Agency Service Center (gigezrate.guard.com/dotnet/mvc/asc/ascheader/home/index), and everyone else is sent to the Customer Service Team. That team is the Contact Us page, which publishes 1-800-673-2465, csr@guard.com, a 570-820-7968 fax and an on-page Customer Service Request form - so csr@guard.com is a general customer-service inbox that GUARD itself designates for loss run requests, not a loss-run-only address. Third parties (an insured's new broker, a premium auditor) should say 'loss run request' explicitly and expect a service rep, not a claims analyst. No turnaround is published for any route. Nothing states what a request must contain; the Customer Service Request form requires only name, phone, email and a comment, with policy or claim number and agent name optional. 'Berkshire Hathaway GUARD Insurance Companies' is a trade name, not an insurer - the paper is AmGUARD, AZGUARD (surplus lines), EastGUARD, NorGUARD or WestGUARD, all at 39 Public Square, Wilkes-Barre PA, all reached through the same channels, so name the issuing company off the dec page but do not look for a separate unit-level contact. Two real guard.com addresses are not loss run routes: claims@guard.com is the FNOL/claims desk on the Claims Center page, and underwriting3@guard.com is a CC address for Connecticut managed-care plan certification forms - an aggregator directory lists the latter as 'NorGUARD loss run requests' on the strength of an anonymous 2019 contribution, and it is wrong. Evidence caveat: guard.com serves this crawler an Azure WAF 403, so every page below was read from Internet Archive snapshots taken 2026-04-11 to 2026-06-10; the contacts are consistent across all of them but re-check before relying on them.
